Formerly Sutter VNA & Hospice and before that Home Hospice of Sonoma County & the oldest hospice in Sonoma County. Sutter Care at Home extends the continuum of health care from hospitals and doctors’ offices directly into patients’ homes, where they receive medical attention, education, and compassionate care needed to recover or live with illness to the fullest extent possible. There focus is to keep families together during a stressful time, and we care for the whole family with social services and emotional support. For many patients, we are all that stands between them and the “revolving door” of repeated hospitalizations, emergency room visits, or nursing facilities .
Sutter Care at Home is a locally based, not-for-profit organization. As such they do not perform for shareholders, but instead, all revenues – including any “profit” earned beyond expenses – are invested back into our mission, especially in patient care, charity care, and community services. Sutter Care at Home is an affiliate of Sutter Health, one of the nation’s leading not-for-profit health care networks of hospitals, physicians, and health care providers.
As part of our community services, we provide a number of bereavement support groups for both children and adults, available free to the community to anyone who has a need.
