The mission of Heartland Hospice is to provide specialized care that meets the physical, emotional, and spiritual needs of patients and loved ones facing the challenges of an advanced illness. The quality of life for those we serve is enhanced by encouraging personal choices, supporting meaningful experiences and preserving dignity. Our staff is actively involved in all aspects of the patient’s care. We collaborate with the patient, family, attending physician, and facility staff (if patient is residing in an extended care facility) to collaborately focus on providing comfort, pain and symptom management, spiritual and emotional support.
As one of the nation’s largest Medicare certified hospice agencies, we are committed to the highest standards of patient care. Heartland Hospice strives to provide care to all who are eligible and need/want hospice care irrespective of ability to pay. We have been serving Sonoma and Marin counties for over 10 years and are proud to be an active part of the community.
