Robert Wood Johnson Foundation Rallying Points was a national program (active 2000 – 2004)dedicated to supporting grassroots efforts to improve quality of care and life through life’s end. Grants received from the RWJF Rallying Point Program enabled the Community Network: to work with our Latino Advisory Council and Sonoma County In Home Support Services Public Authority to create a four-hour Spanish-language “End of Life Issues” pilot training program (2004); to provide planning and support services for the End of Life Care Alliance Steering Committee (2000-2004); and to employ the services of Bo Laurent for services needed to design, build and maintain the website (2004-2006).
