The Pacific Foundation for Medical Care granted the Community Network a part of the seed money needed to start the Journey Project in 1997. In 2011 PFMC granted us funding needed to help further our work educating physicians about end of life planning and care. The Pacific Foundation for Medical Care is a nonprofit organization dedicated to improving patient access to physicians. Through their affiliation with the California Foundation for Medical Care, they are part of one of the largest preferred provider organization (PPO) in California, with more than 35,000 physicians, hospitals, and other health care providers throughout the state.
PFMC serves:
Physicians and other providers who want to join the PFMC network.
Health plans and self-insured employers who want to lease the PFMC network or purchase medical review services.
Employers who want to learn about the Basic Plus health plan.
