Improving Care Through Life's End for People in Residential Care: Challenges & Opportunities

Program: 3:00 - 6:00 PM; Registration: 2:30 -3:00 PM; *Pre-registration required by Oct 6th. Registration packets mailed to all residential care homes in Sonoma County mid-September. $25 for 3 CEUs, BRN, BBS, RCFE and ARF. Others $15 suggested donation at the door.

Who Should Attend. Administrators/Owners and Staff of Assisted Living Facilities, Residential Care Facilities for the Elderly/Board and Care, and Facilities for the Developmentally Disabled others working in those settings.
Participants will improve knowledge and skills in:
Assisting residents & families to clarify end of life care wishes and goals of care; Honoring residents wishes for limited interventions including palliative and comfort care; Understanding the importance of POLST for honoring a person's wishes across the care continuum.
