Sutter Visiting Nurse Association & Hospice

Sutter Visiting Nurse Association (VNA) & Hospice (formerly Home Hospice of Sonoma County) , the oldest hospice in Sonoma County) extends the continuum of health care from hospitals and doctors' offfices directly into patients' homes, where they receive medical attention, education, and compassionate care needed to recover or live with illness to the fullest extent possible. Our focus is to keep families together during a stressful time, and we care for the whole family with social services and emotional support. For many patients, we are all that stands between them and the "revolving door" of repeated hospitalizations, emergency room visits, or nursing facilities .

Sutter VNA & Hospice is a locally based, not-for-profit organization. As such we do not perform for shareholders, but instead, all revenues - including any "profit" earned beyond expenses - are invested back into our mission, especially in patient care, charity care, and community services.Sutter VNA & Hospice is an affiliate of Sutter Health, one of the nation's leading not-for-profit health care networks of hospitals, physicians, and health care providers.
