Pacific Foundation for Medical Care

The Pacific Foundation for Medical Care granted the Community Network a part of the seed money needed to start the Journey Project. The Pacific Foundation for Medical Care is a nonprofit organization dedicated to improving patient access to physicians. Through their affiliation with the California Foundation for Medical Care, they are part of the largest preferred provider organization (PPO) in California, with more than 35,000 physicians, hospitals, and other health care providers throughout the state.
