Program: 3:00 - 5:30 PM; Registration: 2:30 -3:00 PM; Reception After: 5:30 - 6 PM. *Pre-registration required by Oct 6th. Registration packets will be ready Sept. 7th and mailed to all residential care homes in Sonoma County then. $25 for 2.5 CEUs, BRN, BBS, DSS CCL. Others $15 suggested donation at the door.
Who Should Attend. Administrators/Owners and Staff of Assisted Living Facilities, Residential Care Facilities for the Elderly/Board and Care, and Facilities for the Developmentally Disabled.
Participants will improve knowledge and skills in:
Assisting residents & families to clarify end of life care wishes and goals of care; Honoring residents wishes for limited interventions including palliative and comfort care; Understanding the importance of POLST for honoring a person's wishes across the care continuum.
